Sancta Maria is seeking a full-time, compassionate and skilled Social Worker to support the psychosocial needs of residents in our long-term care and skilled nursing facility. You will work in collaboration with another full-time social worker and will be part of an interdisciplinary care team. This position’s main focus is our long-term residents. You will play a vital role in improving the quality of life for our residents through assessment, care planning, counseling, discharge coordination, and ongoing family support.
