Sancta Maria Nursing Facility is pleased to announce that we are participating in a Medicare initiative called Bundled Payments for Care Improvement initiative Model 3.
Medicare is using the Bundled Payments Model 3 to encourage doctors to work closely with providers like Sancta Maria who help patients recover after they are discharged from the hospital, so patients get better care following their hospital stay.
Sancta Maria Nursing Facility is working closely with certain health care providers that will provide care following a resident’s or patient’s hospital stay and extending through the recovery period, as well as hospitals in the event of re-hospitalization. This participation means that in addition to providing residents and patients with the same level of high quality care as in the past, we have agreed to a new payment arrangement with the goal of achieving greater administrative efficiency and more coordinated care.
This new payment delivery model is a part an innovation program offered by Medicare.
MeHI, the Massachusetts eHealth Institute at MassTech, has announced the first round of long-term and post-acute care (LTPAC) grantees from the eQuality Incentive Program (eQIP).
MeHI awarded nearly $1 million in grants to 15 post-acute care organizations that oversee 44 facilities and 6,110 patient beds across Massachusetts. Under the eQIP program, MeHI’s funds will help these Massachusetts facilities invest in innovative health information technologies to improve patient care through enhanced care coordination and efficient care transitions.
“Sancta Maria Nursing Facility is extremely pleased to have been awarded a grant from MeHI,” said Mary Jane Nottonson, Administrator.
“As a provider of short term rehab and long term care, these funds will allow us to expand upon our existing capabilities through the enhanced and innovative use of health information technology to improve care transitions. I commend MeHI for recognizing the vital role of long term and post-acute care facilities in the health care continuum, through this generous grant initiative.”

Overall, life at Sancta Maria is enriched by a range of therapeutic activities and social events designed for people at all functional levels.
On June 18 Sancta Maria had the pleasure of hosting a wine social in celebration of the opening of our new Patient Rehabilitation gymnasium.
Located on the 4th floor, the new rehab gym is centrally located for easy access for Rehab staff and patients alike. A nice, bright, and airy space, the gym is a major upgrade, featuring new equipment and big windows, marking the gym as a go to place for patients striving to go home.
Our celebratory wine tasting was well-attended by Physicians, Case Managers, Nurses and Social Workers, all of whom are true friends of Sancta Maria – and to them we extend a sincere thank you! Many tours were conducted during the event, and all who viewed the 4th floor were very impressed by the expansive rooms and the many resources that allow us to take care of our patients so well.

Mother’s Day at Sancta Maria Nursing Facility saw Patients, families and staff celebrate with a Mother’s Day Brunch.
It was a beautiful Summer day in the Springtime. Temperatures were in the high eighties and everyone was in a celebratory mood. Most everyone took advantage of the day and enjoyed the good weather.
The grounds at Sancta Maria are quite lovely these days and it made for a beautiful afternoon. Activities Director Amanda Benduzak and Food Service Director Joanna Mitrano were instrumental in the planning and implementation of the Brunch. Styltangfehuku Administrator Mary Jane Nottonson and Director of Employee Relations Jeanne Citarella were also in attendance.
One resident summed-up the day very well by saying that, “It was a great day and everyone had a great time!”
On April 15, 2015, residents of Sancta Maria Nursing Facility in Cambridge, MA celebrated One Boston Day alongside students and faculty from Cambridge Rindge and Latin School.
In honor of the day’s theme of kindness, hand-crafted cedar wood planters built by the school’s carpentry students were presented to the residents of Sancta Maria. Residents and staff of Sancta Maria, and Sisters from the Daughters of Mary of the Immaculate Conception as well as faculty and students from Cambridge Rindge and Latin High School all came together to celebrate this generous gift by planting flowers and vegetable seeds in the new wheelchair accessible, movable planters.
Materials were purchased through a grant written to the Friends of Cambridge Rindge and Latin School, and faculty generously donated the soil and the seeds.

Easter Sunday Mass was celebrated here at Sancta Maria Nursing Facility in our chapel, which is a serene and comfortable place for prayer and quiet reflection.
Father Francis’ sermon involved references to rolling away the stone as the followers of Christ did. He handed out stones and suggested that we all need to learn about rolling away stones to start a new beginning.
The mass was one of several ceremonies scheduled by our active community here at Sancta Maria. ospinasilveyhe44.blogspot.com
